Key Responsibilities
  • Lead a team of Care Assistants in providing high-quality care and support to residents
  • Develop and implement personalized care plans in collaboration with residents, families, and healthcare professionals
  • Conduct regular assessments and reviews to ensure the effectiveness of care plans
  • Collaborate with the Registered Nurse and Care Manager to ensure seamless care delivery
  • Provide training and guidance to Care Assistants to enhance their skills and knowledge
  • Maintain accurate records of care delivery and resident progress
Requirements
  • Level 2 Diploma in Health and Social Care or equivalent
  • Background in person-centered care planning and knowledge of caring for people with dementia
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong organizational and time management skills
