About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with mental health needs in our community.

Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments and risk assessments to identify individuals' needs and develop effective care plans.
  • Deliver evidence-based interventions, including brief psychosocial interventions, to promote recovery and improve mental health outcomes.
  • Work collaboratively with individuals, carers, and other healthcare professionals to develop and implement personalized care plans.
  • Provide expert advice and guidance to colleagues and other stakeholders to ensure high-quality care and support.
  • Contribute to the development and implementation of service development initiatives to improve mental health services and outcomes.
Requirements
  • Registered Mental Health Nurse (RMN), Registered Mental Health Practitioner (RNMH), Social Worker, or equivalent qualification.
  • Significant post-registration experience in mental health care, including experience of working with individuals with complex needs.
  • Excellent communication and interpersonal skills, with the ability to form productive working relationships with individuals, carers, and other healthcare professionals.
  • Comprehensive knowledge of mental health legislation, policies, and guidelines, including safeguarding and child protection legislation.
  • Ability to work autonomously and as part of a multidisciplinary team, with a strong commitment to delivering high-quality care and support.
