Main Duties- Provide high-quality nursing care to children with varying health needs, including those with life-limiting or life-threatening conditions.
- Work collaboratively with the Community Children's Nursing Team to develop and implement individualized care plans, ensuring seamless transitions between hospital and community settings.
- Facilitate the development of the extended role of the Community Children's Nursing Team, providing teaching, support, and resources to colleagues.
- Communicate complex and sensitive information to parents and carers, ensuring they are confident and competent in caring for their child at home.
- Manage client caseloads, work autonomously, and delegate tasks to junior staff members as needed.
- Contribute to the smooth running of the team, attending discharge planning meetings and providing input on patient care.
