Job Summary

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at Croydon Health Services NHS Trust. As a key member of our multidisciplinary team, you will provide expert clinical practice in the management of diabetes for community/housebound patients in Croydon.

Main Responsibilities
  • Provide specialist clinical input for the management of complex diabetes patients
  • Develop and deliver education and training programs for healthcare professionals and patients
  • Work closely with the multidisciplinary team to ensure seamless care for patients with diabetes
  • Contribute to the development of evidence-based clinical nursing practice in diabetes care
  • Collaborate with other healthcare professionals to provide comprehensive care for patients with diabetes

Person Specification
  • Registered Adult Nurse with a recognized Adult Diabetes qualification
  • Extensive experience in Adult Diabetes nursing, including working as a Diabetes Specialist Nurse
  • Ability to provide support, education, and specialist input for the management of complex patients and improved management of diabetes
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
