Job Summary

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at Croydon Health Services NHS Trust. As a key member of our multidisciplinary team, you will provide expert clinical practice in the management of diabetes for community/housebound patients in Croydon.

Main Responsibilities
  • Provide specialist clinical input for the management of complex diabetes patients
  • Develop and deliver education and support programs for patients and their families
  • Work closely with the multidisciplinary team to ensure seamless care for patients
  • Contribute to the development of evidence-based clinical nursing practice in diabetes care
  • Collaborate with other healthcare professionals to provide coordinated care

Person Specification
  • Registered Adult Nurse
  • Holds recognised Adult diabetes qualification
  • Extensive experience in Adult diabetes nursing, including working as a Adult diabetes specialist nurse
  • Able to provide support, education and specialist input for the management of more complex patients and improved management of diabetes
