Job Summary

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at Croydon Health Services NHS Trust. As a key member of our multidisciplinary team, you will provide expert clinical practice in the management of diabetes for community/housebound patients in Croydon.

Main Responsibilities
  • Provide and manage expert specialist clinical practice in diabetes care for community/housebound patients.
  • Develop and deliver evidence-based clinical nursing in line with current research and guidelines.
  • Work closely with the wider multidisciplinary team to prevent hospital admission, support early discharge, and reduce GP contact.
  • Provide structured education for people with diabetes in community-based services.
  • Initiate and monitor diabetes medication and insulin, as well as titrate/optimise medication accordingly.
About the Role

This is a full-time position that requires a high level of expertise in diabetes care. You will have the opportunity to work closely with colleagues in hospital, community, and primary care to ensure that people with diabetes receive high-quality integrated care.

Person Specification
  • Registered Adult Nurse with a recognised Adult diabetes qualification.
  • Extensive experience in Adult diabetes nursing, including working as a Diabetes Specialist Nurse.
  • Able to provide support, education, and specialist input for the management of complex patients and improved diabetes management.
