Job Summary

We are seeking a highly skilled Diabetes Specialist Nurse to join our team at Croydon Health Services NHS Trust. As a key member of our multidisciplinary team, you will provide expert clinical practice in the management of diabetes for community/housebound patients in Croydon.

Main Responsibilities
  • Provide and manage expert specialist clinical practice in diabetes care for community/housebound patients.
  • Develop and deliver evidence-based clinical nursing in line with current research and guidelines.
  • Work closely with the wider multidisciplinary team to prevent hospital admission, support early discharge, and reduce GP contact.
  • Provide structured education for people with diabetes in community-based services.
  • Initiate and monitor diabetes medication and insulin, as well as titrate/optimise medication accordingly.
About the Role

This is a full-time position that requires a Registered Adult Nurse with a recognised Adult diabetes qualification. You will have extensive experience in Adult diabetes nursing, including working as a Diabetes Specialist Nurse. You will be able to provide support, education, and specialist input for the management of complex patients and improved diabetes management.
