Job Summary

We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team at Croydon Health Services NHS Trust. As a key member of our multidisciplinary team, you will provide expert clinical practice in the management of diabetes for community/housebound patients in Croydon.

Main Responsibilities
  • Provide and manage expert specialist clinical practice in diabetes care for community/housebound patients.
  • Develop and deliver evidence-based clinical nursing in line with current research and guidelines.
  • Work closely with the wider multidisciplinary team to prevent hospital admission, support early discharge, and reduce GP contact.
  • Provide structured education for people with diabetes and their families.
  • Lead nurse-led clinics and joint clinics in community locations to support the management of people with diabetes.
Key Skills and Qualifications
  • Registered Adult Nurse with a recognized Adult diabetes qualification.
  • Extensive experience in Adult diabetes nursing, including working as a Diabetes Specialist Nurse.
  • Ability to provide support, education, and specialist input for the management of complex patients and improved diabetes management.
  • Excellent communication and interpersonal skills.
