Job Title: Senior Community Dementia Nurse

We are seeking a highly skilled and compassionate Senior Community Dementia Nurse to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Dementia Intensive Support Service, you will play a vital role in providing high-quality, evidence-based care to individuals living with dementia.

About the Role

As a Senior Community Dementia Nurse, you will be responsible for providing rapid assessments, developing and implementing care plans, and working collaboratively with our multidisciplinary team to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized care plans in collaboration with service users, carers, and other healthcare professionals.
  • Provide high-quality, evidence-based care to individuals living with dementia, including medication management, symptom control, and support with daily living activities.
  • Work closely with our Functional Intensive Response and Support Team (FIRST) to prevent unnecessary hospital admissions and support early discharge.
  • Supervise and mentor junior team members, providing guidance and support to ensure the delivery of high-quality care.
  • Participate in multidisciplinary meetings and contribute to the development of care plans and service improvements.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a strong background in dementia care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with service users, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality care.
  • Current UK driving license and access to a car for work purposes.
