We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our team in Mid Essex. As a Band 6 Community Nurse, you will be responsible for providing high-quality, evidence-based care to patients with dementia and other mental health conditions.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to support patients in their homes and communities.
- Work collaboratively with multidisciplinary teams, including social workers, psychologists, and other healthcare professionals, to ensure seamless care coordination.
- Provide crisis intervention and support to patients and their families, using evidence-based models and best practices.
- Supervise and mentor junior staff members, promoting professional development and growth within the team.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care and outcomes.
- Registered Mental Health Nurse or Registered General Nurse with a strong background in community mental health.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Knowledge of dementia and mental health conditions, including assessment, diagnosis, and treatment options.
- Experience in crisis intervention and management, with a focus on patient-centered care and safety.
- Ability to work independently and as part of a team, with a strong commitment to professional development and continuous learning.
