About the Role

We are seeking a highly skilled and dedicated Senior Community Nurse to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Dementia Intensive Support Service, you will play a vital role in providing high-quality, evidence-based care to patients with dementia and their families.

Key Responsibilities
  • Assess, plan, implement, and evaluate packages of care for patients with dementia, working closely with the Advanced Nurse Practitioner and other multidisciplinary team members.
  • Provide crisis interventions and home treatment to prevent hospital admissions, using your expertise in dementia and frailty in older adults.
  • Work collaboratively with key partner organizations, service users, carers, clinicians, and other practitioners to deliver a 7-day a week service for 365 days a year.
  • Supervise and manage junior team members, providing guidance and support to ensure the delivery of high-quality care.
  • Develop and maintain strong relationships with service users, carers, and other stakeholders, ensuring their needs are met and their voices are heard.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a good standard of written and spoken English.
  • Postgraduate qualification or commitment to work towards a non-medical prescribing qualification.
  • Excellent IT skills, including Word, Excel, PowerPoint, and Outlook.
  • Current UK driving license and access to a car for work purposes.
