About the Role

We are seeking a highly skilled and experienced Senior Community Nurse Specialist to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Community Mental Health team, you will play a vital role in providing high-quality, evidence-based care to individuals with dementia and other mental health conditions.

Key Responsibilities
  • Provide rapid assessments and interventions for individuals in crisis situations, working closely with multidisciplinary teams to develop and implement effective care plans.
  • Work collaboratively with colleagues to provide holistic care, including assessments, risk assessments, and care planning.
  • Supervise and manage junior team members, providing guidance and support to ensure the delivery of high-quality care.
  • Develop and maintain strong relationships with service users, carers, and other stakeholders to ensure effective communication and collaboration.
  • Contribute to the development and implementation of service improvements, working closely with colleagues to identify areas for growth and development.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a strong background in community mental health.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Strong assessment and planning skills, with the ability to prioritize and manage multiple tasks and responsibilities.
  • Experience of working in a community mental health setting, with a strong understanding of dementia and other mental health conditions.
  • Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality care.
