About the Role

We are seeking a highly skilled and experienced Senior Community Nurse Specialist to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Community Mental Health team, you will play a vital role in providing high-quality, evidence-based care to patients with dementia and other mental health conditions.

Key Responsibilities
  • Provide rapid assessments and interventions for individuals with dementia or suspected dementia in crisis situations.
  • Work collaboratively with multidisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
  • Supervise and manage junior team members, providing guidance and support to ensure the delivery of high-quality care.
  • Contribute to the development and implementation of service improvements, ensuring that our services are responsive to the needs of our patients and communities.
  • Participate in multidisciplinary meetings and contribute to the development of care plans and service strategies.
Requirements
  • Registered Mental Health Nurse or Registered General Nurse with a strong background in community mental health.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
  • Knowledge of dementia and mental health conditions, including assessment, diagnosis, and treatment options.
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
  • Current UK driving license and access to a car for work purposes.
