About the Role

We are seeking a highly skilled and experienced Community Mental Health Practitioner to join our Dementia Intensive Support Service team. As a Band 6 practitioner, you will be responsible for providing rapid assessments and interventions for individuals living with dementia or suspected dementia in crisis situations.

Key Responsibilities

  • Assess and develop care plans for individuals with dementia or suspected dementia in crisis situations
  • Work collaboratively with multidisciplinary teams to provide high-quality, evidence-based care
  • Provide supervision and guidance to junior team members
  • Contribute to the development and implementation of service policies and procedures

Requirements

  • Registered Mental Health Nurse or Registered General Nurse with a good standard of written and spoken English
  • Experience of working in mental health services, particularly in crisis intervention and dementia care
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
