About This Role

We are seeking a compassionate and experienced Carer Peer Support Worker to join our dedicated team at Elmwood Ward. In this role, you will play a vital part in providing emotional and practical support to families and carers of patients with dementia.

Your firsthand experience caring for a loved one diagnosed with dementia will be invaluable as you connect with and understand the unique challenges faced by these individuals. You will act as a source of hope, guidance, and encouragement, helping them navigate the complexities of hospital care and beyond.

Key Responsibilities

  • Deliver personalized support sessions tailored to the individual needs of carers, encompassing information sharing, concern addressing, self-care enhancement, well-being promotion, and guidance towards external support services.
  • Share your personal experiences in a way that is both beneficial and respectful, maintaining clear boundaries.
  • Champion the principles of the Triangle of Care by organizing monthly peer support groups and assisting with Family Support Workshops.
  • Conduct thorough research to identify relevant support services for families and carers.
  • Maintain the carer information station in collaboration with the Elmwood Ward team.
  • Plan and prepare for Carers Week and Carer's Rights Day, providing valuable information to families both on and off the ward.
  • Collaborate closely with the team, keeping them informed about carer matters and assisting in staff training regarding carer involvement.
  • Participate actively in online carer peer and carer-led meetings.
  • Complete IMROC Peer Support training and demonstrate a thorough understanding of peer support principles.
  • Develop a comprehensive understanding of the impacts on families and carers, as well as relevant legal frameworks affecting carers.
  • Successfully complete Carer Communication Plans.
