Job Description

Role Overview

Concept Resources is seeking a highly motivated and compassionate Recovery Housing Support Specialist to join our team. As a key member of our substance use services team, you will play a vital role in providing flexible support to individuals experiencing problems with substance use, domestic abuse, and homelessness.

Key Responsibilities

  • Implement housing support plans based on individual needs, ensuring a tailored approach to meet each person's unique circumstances.
  • Conduct risk assessments and develop risk management strategies to ensure the safety and security of service users.
  • Empower individuals to live independently by supporting them in developing life management skills, such as budgeting, cooking, and maintaining their dwelling.
  • Provide housing support to maintain the security and safety of service users' dwellings.
  • Support individuals in managing their health and well-being, including access to general health advice and signposting to other services.
  • Plan service delivery contact flexibly to meet individual needs, adopting a proactive approach to engagement to maximize service accessibility.
  • Signpost service users to providers of specialist advice, such as CAB, welfare rights, and advocacy services.
  • Encourage and support individuals in maintaining good relations and avoiding disputes.
  • Take the lead professional role where required, supporting colleagues from other agencies, including rapid response to crises.
  • Provide assertive and persistent support to individuals and agencies to ensure effective engagement and achievement of personal goals and aspirations.
  • Accurately record and maintain service users' progression, records, and plans.
  • Ensure individuals have regular, up-to-date copies of their support plan and safety plan.
  • Record activity and results, accurately and promptly, to support project evaluation and funding requirements.
  • Attend supervision regularly and commit to continuous professional development.
