**Job Summary**

Creative Support is a leading provider of person-centred support services for individuals with learning disabilities, mental health needs, physical disabilities, and other support needs. We are seeking a highly motivated and compassionate Support Professional to join our team in Middlesbrough.

**Key Responsibilities**

  • Provide personal care, practical, and emotional support to individuals with learning disabilities/mental health needs.
  • Identify and access opportunities for social inclusion with service users.
  • Work with service users in a recovery-focused way, in line with their individual support plans.
  • Collaborate with service users, families, and involved professionals to provide a consistent and coordinated service that meets the needs of service users.
  • Assist the line manager with the day-to-day running of the service.

**Requirements**

We are looking for a warm and empathetic individual who is able to work in a person-centred way. A full driving license is essential for this role. The successful candidate will have the opportunity to work in an environment that challenges and encourages personal and professional development.
