Primary Care Nurse Specialist

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at Practice Plus Group. As a Primary Care Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Deliver a nurse-led service, preventing hospital admissions and providing evidence-based care to patients.
- Undertake medication rounds, dispense controlled medication, and attend emergency response situations.
- Assess patients, undertake reception screening, and use your skills and knowledge to deliver nurse-led clinics to your patient group.
- Contribute to our nursing strategy and vision, providing outstanding evidence-based care, delivering non-judgmental care, inspiring excellence, promoting best practice, and sharing knowledge and expertise.
- Registered Nurse (Adult) with a current professional qualification (NMC).
- Understanding of the complexities of multi-disciplinary and multi-agency working.
- A non-judgmental and compassionate approach.
- Understanding of clinical governance, health and safety, QOF, EBP, and national standards.
