Key Responsibilities- Provide clinical leadership for the primary care team, ensuring patient safety and the management of complex cases.
- Assess, diagnose, treat, and discharge or refer patients, independent of medical intervention.
- Develop and implement evidence-based care plans, ensuring patient-centered and personalized care.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and effective communication.
- Contribute to the development and implementation of clinical policies and procedures, ensuring compliance with regulatory requirements.
- Registered professional with current registration (NMC).
- Evidence of Continued Professional Development.
- Masters in Advanced Practice or equivalent.
- Recognised prescribing qualification.
- Understanding of the complexities of multi-disciplinary and multi-agency working.
