Primary Care Nurse

About the Role
We are seeking a skilled and compassionate Primary Care Nurse to join our team at Practice Plus Group. As a Primary Care Nurse, you will work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
Key Responsibilities
- Undertake medication rounds and dispense controlled medication
- Attend emergency response situations and assess patients
- Undertake reception screening and use your skills and knowledge to deliver nurse-led clinics to your patient group
- Contribute to our nursing strategy and vision to provide outstanding evidence-based care to our patients
- Deliver non-judgmental care that makes a real difference, as defined throughout our bespoke Health in Justice competency framework
- Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of
- Promote best practice, clinical supervision, evidence-based care, and continuous quality improvement, embedding a culture of shared learning
- Share knowledge, skills, and expertise to ensure safe care, building strong multi-disciplinary teams and supporting each other to do a great job

Requirements
- Registered nurse Adult
- Current professional qualification (NMC)
- Evidence of Continued Professional Development
- Understanding of the complexities of multi-disciplinary and multi-agency working
- Understanding of clinical governance, health and safety, QOF, EBP, and national standards
Desirable Qualifications and Experience
- Experience in a hospital, community, or prison environment
