About the Role

Cornwall Partnership NHS Foundation Trust is seeking a highly skilled and dedicated individual to join our Community Mental Health Team based in Liskeard. As a Senior Community Mental Health Nurse, you will play a vital role in delivering high-quality care to individuals experiencing severe and enduring mental health problems.

Key Responsibilities

  • Assess and manage a defined caseload of patients with complex mental health needs
  • Develop and implement individualized care plans in collaboration with patients, families, and multidisciplinary teams
  • Provide expert clinical leadership and supervision to junior staff members
  • Contribute to the development and implementation of service policies and procedures
  • Participate in quality improvement initiatives and contribute to the evaluation of service effectiveness
