Senior Community Mental Health Nurse

We are seeking a highly skilled and motivated Senior Community Mental Health Nurse to join our team at Cornwall Partnership NHS Foundation Trust.
The successful candidate will be responsible for assessing care needs and providing clinical management to a defined caseload within our Mental Health Service.
Key responsibilities include developing and implementing programs of care, utilizing dialog+ and other outcome measures, and developing the interface between primary and secondary care services.
The ideal candidate will have extensive experience working with mental health patients, demonstrate clinical leadership skills, and possess a sound working knowledge of the Mental Health Act of 1983 and other mental health legislation.
Key Qualifications and Experience- Current professional registration with the HCPC
- Evidence of post-basic training
- Extensive experience of working with mental health patients
- Demonstrate clinical leadership skills
- Professional qualification in OT, Social work, or RMN
