About the Role

Cornwall Partnership NHS Foundation Trust is seeking a highly skilled and motivated individual to join our Community Mental Health Team in Liskeard. As a key member of the team, you will be responsible for delivering high-quality care to individuals experiencing severe and enduring mental health problems.

Main Responsibilities

Assess and manage a defined caseload within the Cornwall Partnership NHS Foundation Trust Mental Health Service, ensuring effective care planning and coordination.

Develop and implement programs of care, utilizing dialog+ and other outcome measures, to promote recovery and independence.

Establish strong relationships with primary and secondary care services to provide focused and coordinated packages of care for patients and carers.

Requirements

Current professional registration with the HCPC as a Registered Mental Health Nurse (RMN)

Post-basic training and extensive experience working with mental health patients

Clinical leadership skills and ability to supervise staff

Professional qualification in mental health nursing (OT, Social worker, RMN)

Knowledge and Skills

Sound working knowledge of the Mental Health Act 1983 and other mental health legislation

Ability to work collaboratively with multidisciplinary teams and stakeholders
