Job Title: Mental Health Support Worker

Job Summary:

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting individuals with complex needs, disabilities, and long-term conditions to live independently in their own homes.

Key Responsibilities:

  • Provide person-centered support to individuals with mental health conditions, learning disabilities, and complex needs.
  • Assist with daily living tasks, such as personal care, medication management, and household chores.
  • Support individuals to engage in activities that promote their physical and mental well-being, such as swimming, outdoor walks, and cooking.
  • Work collaboratively with the support team to develop and implement individualized care plans.
  • Monitor and report any changes in an individual's condition or behavior to the support team.

Requirements:

  • Full UK driving license (essential)
  • Social care experience, ideally within mental health and with some experience of supporting individuals with schizophrenia.
  • Experience providing encouragement to individuals with complex health needs and learning disabilities.
  • Experience supporting individuals with behaviors that may be considered challenging.
  • Experience implementing detailed care plans and risk assessments.
  • Current Right to Work in the UK and have been employed in the UK for at least 6 months.
  • Enhanced DBS check and satisfactory references.
