Position Overview

We are seeking a skilled Healthcare Coordination Specialist to join our reputable domiciliary care organization. This role is crucial for ensuring the well-being of our diverse clientele across various stages of health and recovery.

Key Responsibilities
  • Scheduling: Develop and manage daily rotas for care staff, adjusting schedules as necessary to address unforeseen circumstances such as illness or emergencies.
  • Care Plan Management: Create, update, and maintain comprehensive care plans while ensuring accurate client information is recorded in our digital management systems.
  • Client Liaison: Engage with clients, their families, and relevant stakeholders to effectively support the needs of our service users.
  • Domiciliary Care Support: Provide care services as needed, which may include evenings, weekends, and holidays.
  • On-Call Duties: Occasionally manage the on-call service during weekends, with compensatory time off in the following days.
  • Medication Oversight: Responsible for medication ordering and conducting reviews with care staff.
  • Additional Tasks: Undertake various ad-hoc responsibilities to support the organization, service users, and team members.
Qualifications

The ideal candidate will possess a Level 3 NVQ/Diploma in Health & Social Care and have experience with care management systems, such as CarePlanner. A full, clean driving license and personal vehicle are essential to meet the dynamic needs of our clients.
