Job Title: Team Leader Support Worker

Job Summary:

We are seeking a highly skilled and experienced Team Leader Support Worker to join our team at Libertatem Healthcare Group. As a Team Leader Support Worker, you will be responsible for providing high-quality care and support to our clients, as well as managing a team of support workers to ensure the delivery of excellent care services.

Key Responsibilities:

  • Provide leadership and support to a team of support workers to ensure the delivery of high-quality care services.
  • Manage the day-to-day operations of the care team, including rota management, annual leave, and supervision.
  • Develop and implement care plans that meet the individual needs of our clients.
  • Work closely with the healthcare coordinator and clinical lead nurse to ensure that all care services are delivered in accordance with company policies and procedures.
  • Ensure that all care services are delivered in a safe and effective manner, and that all risks are identified and mitigated.
  • Provide emotional support and guidance to our clients, and promote their independence and well-being.
  • Collaborate with other healthcare professionals to ensure that all care services are delivered in a coordinated and effective manner.
  • Ensure that all care services are delivered in accordance with the principles of safeguarding, and that all concerns are reported in line with company procedures.

Requirements:

  • Experience of working in a care setting, with a focus on providing high-quality care and support to individuals with complex needs.
  • Proven leadership and management skills, with the ability to motivate and support a team of support workers.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and other healthcare professionals.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of our clients.
  • Knowledge of care policies and procedures, including safeguarding and data protection.
