About the Role:
We are seeking a highly skilled and dedicated Community Learning Disability Nurse to join our team at Southern Health NHS Foundation Trust. As a Community Learning Disability Nurse, you will play a vital role in providing high-quality care to individuals with learning disabilities and their families.
Key Responsibilities:
- Assess, plan, implement, and evaluate the nursing care needs of individuals with learning disabilities.
- Work collaboratively with multidisciplinary teams to provide comprehensive care and support.
- Develop and maintain effective relationships with service users, their families, and carers.
- Contribute to the development and implementation of care plans that meet the individual needs of service users.
- Supervise and mentor junior staff members to ensure high-quality care and support.

Requirements:
- Registered Learning Disabilities Nurse qualification.
