The role of the Healthcare Assistant (HCA) is to provide high-quality care to patients under the supervision of a Registered Nurse. The HCA will maintain a clean and welcoming environment for patients and relatives, working collaboratively with the Multi-Disciplinary Team (MDT).
Main Duties- Provide personalized care to patients in accordance with local and national guidelines.
- Perform essential nursing care to a high standard, meeting the individual needs of patients.
- Assist patients with personal hygiene, including hair, teeth, and nail care, and under supervision, assist with oral hygiene for conscious patients.
- Assist with bathing in bed and in the bathroom, using mechanical aids and bed appliances as required.
- Assist in the care of pressure areas and incontinent patients.
- Complete Core Skills Training, the Care Certificate, and Healthcare Assistant Induction.
- Demonstrate compassion by recognizing and responding to individual needs in a warm, thoughtful, and sensitive manner.
- Support the delivery of high-quality end-of-life care, including care of the deceased and bereaved relatives.
- Maintain patient privacy, dignity, and respect at all times.
- Proactively engage with patients, relatives, and carers to seek their views and gain cooperation and agreement.
- Report or escalate concerns about any patient's condition immediately to a Registered Nurse.
- Identify and report any patient needs not included in the care plan.

- Introduce self to patients at the start of each shift.
- Welcome visitors in a manner that promotes a positive image of self and team.
- Ensure compliance with Trust Uniform Policy.
- Take samples from patients for testing or dispatching after appropriate training and competency assessments.
- Ensure that any samples taken are dispatched as per Trust procedures.
- Ensure that samples obtained are stored correctly while awaiting collection and take/send samples to pathology as directed.
- Complete all documentation correctly.
- Assist patients to meet nutritional and hydration needs in line with their individual care plan.
- Ensure timely completion of menu cards and provide assistance to patients as required.
- Prepare patients for mealtimes in a timely manner where required.
- Assist patients with eating and drinking, providing adaptive aids as required.
- Prepare and administer beverages for patients in a timely manner.
- Weigh patients accurately as directed and record results.
- Food/fluid charts are fully completed in a timely manner to Trust standards.
- Assist to maintain patient safety, conforming to health, safety, and security legislation, policies, procedures, and guidelines.
- Uphold uniform policy at all times.
- Ensure nurse call systems are answered in a timely manner.
- Demonstrate the correct use of cot slides, blue trays, and wet floor signs.
- Be compliant with moving and handling update training.
- Report all accidents, incidents, near misses, faulty equipment, environmental issues immediately as per Trust Policy.
- Assist in maintaining standards of care to improve the patient experience and outcome.
- Ensure patients are safe, comfortable, and their care needs are met.
- Take an active part in ward handovers.
- Participate in audit and evaluation programs as requested.
- Makes constructive suggestions to improve services.
- Challenge or report poor standards of care or inappropriate behavior and escalate concerns about patient safety and well-being.
- Use surveys to gain patient feedback about their experience of care.
- Undertake intentional rounding and documentation as requested to meet ward standards.
- In the absence of ward clerks, participate in filing of notes, collection/delivery of notes from and to other departments, admit onto PAS system, and update EPTS if requested.

- Unpack linens and store to maintain stock levels in a timely manner.
- Ensure that ward and utility areas are kept clean and tidy.
- Make beds and change them in a timely manner to keep patients clean and comfortable and bed areas correctly prepared for new admissions and complete bedside checklist.
- Carry out basic administration tasks in the absence of the ward clerk, e.g., filing, ordering, checking stock levels, printing patient wristbands.
- Keep ward equipment cleaned and labeled, completing ward cleaning checklists, e.g., commodes, baths, and lockers.
- Help patients with the care of their personal belongings.
- Report faulty equipment for repair promptly.
- Order and cancel pressure-relieving equipment as directed.
- Complete mattress audits, checking mattresses are fit for purpose.
- Dispose of clinical waste and used linen as per Trust Policy.
- Ensure that telephones are answered stating name, role, and department within 6 rings where possible.
- Report outcomes of care activities in a timely manner, with documentation complete and accurate.
- Transfer patients to other wards, clinics, departments, or hospitals and undertake chaperon and escort duties in adherence with the Trust's Transfer Policy.
- Communicate all relevant information to Registered Nurses in a professional and timely manner.

- Mandatory training as outlined by the Trust is up to date.
- Meet National HCA Code of Conduct (DH 2013).
- Ensure all learning and competencies are completed for use of any equipment used within your role.
- Once airway competent and training completed and standards are maintained, then progression to Band 3.
- Actively try to resolve issues and complaints at local level and then report the outcome to line manager.
- Give clear and concise advice to people on the procedures in place, e.g., PALS, complaints.
- Resolve potential conflicts as per conflict resolution guidance and the promotion of zero tolerance.
- Report incidents of violence or aggression immediately in order to seek help and support.
- Participate in the rehabilitation of patients in hospital.
- Participate in the safe transfer of patients.
- Support and encourage patients to be active participants in their care and recovery plans, e.g., promoting independence with dressing, undressing, taking to the toilet, and using commodes.
- Transfer patients to other wards, clinics, departments, or hospitals and undertake chaperon and escort duties in adherence with the Trust's Transfer Policy.
- Changing intra-venous infusions.
- Giving medical information to patients or relatives.
- Gastric tube feeding.
- Responsibility for the care of an unconscious patient.
- Giving medicine or drugs – orally, by injection, inhalation, or rectal administration.
- Aseptic techniques.
- Assessment of nursing needs, planning of care, and evaluation of nursing care.
