Nurse Specialist

Job Summary:
We are seeking a highly skilled and experienced Nurse Specialist to join our team at Rotherham Doncaster and South Humber NHS Foundation Trust. The successful candidate will be responsible for providing expert care to patients with tissue viability and wound care needs.
Key Responsibilities:
- Participate in the triage of referrals and process within set Key Performance Indicators.
- Work as part of a team delivering patient-centred, evidence-based holistic care within the specialist area of tissue viability, wound care, and lymphoedema.
- Work with primary, secondary care, and nursing home staff and the multi-disciplinary team offering advice and educational input.
- Provide care in a variety of settings, including clinics, wards, hospice, care homes, supported living, and patients' own homes.
- Engage with partner organisations to promote the service and develop effective pathways for patients.
- Be responsible for the assessment, planning, implementation, and evaluation of individualised evidence-based care with minimal ongoing supervision of a senior qualified clinician.
- Promote the health of patients and assist in the prevention of secondary complications of patients with lymphoedema and tissue viability problems, through enabling and encouraging independence, provision of support, facilitation of advice and information, and education and training.
- Liaise with and advise community nursing staff/care home staff and other healthcare professionals to provide specialist assessment and personalised care planning and intervention to all patients referred to the service.
