Job Summary:

  • We are seeking an experienced Practice Nurse Specialist to join our team, responsible for General practice chronic disease management, child/adult vaccinations, wound care, and cytology.
  • Competency in QuoF chronic disease reviews and immunisation is essential.
  • Flexible working hours over 5 days with PM sessions across main and branch sites and ample overtime available to cover colleagues' holidays.
  • Preference for Practice Nurses with Non-Medical Prescribing and Minor Ailments certificates.
  • NMC registration and UK work visa required for overseas candidates.

Main Responsibilities:

  • Chronic Disease Management, including Asthma, COPD, Hypertension, and lifestyle support.
  • Performing routine patient care tasks directed by senior staff.
  • Assisting in minor surgeries, ECGs, Venepuncture, and Travel Clinic procedures.
  • Maintaining equipment, rooms, and stocking supplies.
  • Participating in administrative tasks and maintaining accurate records.
  • Attending Practice meetings and fulfilling delegated duties.

Requirements:

  • Registered First Level Nurse.
  • Clinical Supervision.
  • Minimum 2 years post-registration experience in Chronic Disease Management.
  • Knowledge of national and local guidelines for patient care.
  • Experience in primary and community nursing and quality initiatives.

Essential Skills:

  • Clinical Skills in cervical cytology, immunisation, ear care, minor surgery, and wound care.
  • Experience in teaching and mentorship in a clinical setting.
  • Chronic Disease Management experience.
