Position Overview

The NHS Hertfordshire and West Essex Integrated Care Board is seeking a qualified nurse with extensive expertise in supporting individuals with Learning Disabilities (LD). The ideal candidate will have a background in specialized or community healthcare settings and possess a thorough understanding of the Continuing Healthcare (CHC) framework.

This role presents a unique opportunity to shape and enhance the ICB's approach to understanding and addressing the needs of individuals with LD.

Key Responsibilities

The successful candidate will serve as the case manager for individuals within the CHC caseload who have a Learning Disability. Responsibilities will include:

  • Conducting comprehensive assessments for CHC eligibility.
  • Designing tailored care packages for eligible individuals.
  • Utilizing advanced communication and interpersonal skills to engage effectively with individuals and their families, ensuring their active participation throughout the care process.

Given the geographical diversity of the individuals served, the candidate will be expected to conduct assessments and reviews beyond local boundaries.

Qualifications and Experience

Essential Requirements:

  • Registered Nurse (Adult or Mental Health) with current registration.
  • Degree-level education in a relevant field or equivalent experience in a specialized area.
  • Post-graduate qualification in Management Studies or a related discipline.
  • Experience working within the NHS or a public sector organization.

Knowledge and Skills

Essential Competencies:

  • Proven experience in identifying and addressing safeguarding issues.
  • Significant experience navigating politically sensitive environments.
  • Demonstrated ability to coordinate projects in complex and challenging settings.
  • Experience in risk management and reporting.
