Become a Vital Part of Our Care Coordination Team

This role offers a fulfilling office-based experience with a stable salary, primarily operating during standard business hours, Monday to Friday. On-call duties may be required, with additional compensation provided.

  • Assist in care consultations, service evaluations, and the formulation of client support strategies.
  • Oversee staff supervisions and conduct quality assessments for clients.
  • Develop, update, and assess client care plans while supporting digital care planning initiatives.
  • Keep accurate records of clients and Care Professionals utilizing specialized software.
  • Facilitate introductions between clients and Care Professionals.
  • Build and maintain positive relationships with current and prospective clients, their families, and other professionals involved.
  • Conduct client assessments and implement necessary follow-up actions.
  • Provide guidance and support to Care Professionals.
  • Assist the Client Experience team in their initiatives.
  • Maintain regular communication with clients and Care Professionals.
  • Perform additional tasks as directed by your supervisor.
  • Participate in on-call duties as needed, adhering to company policies.
  • Execute any other essential responsibilities that contribute to the success of the business.

Job Types: Permanent, Full-time

Starting Salary: From £23,500 per annum

Qualifications:

  • Possess a Level 3 NVQ in Health and Social Care or equivalent, or be willing to pursue this qualification.
  • Experience in the care sector providing a range of personal care services.
  • Ability to manage E-MAR sheets and medications in a supervisory role.
  • Commitment to delivering high-quality care and promoting clients' independence.
  • Knowledge of relevant health and social care legislation and regulations.
  • Strong communication skills, capable of building rapport quickly.
  • Comfortable with care management technology and able to provide support to Care Professionals.
  • Respect for confidentiality and understanding of legal boundaries.
  • Proficient in IT systems, with familiarity in Microsoft Office or Google Suite, and adaptable to new technologies.
  • Valid driver's license and access to transportation for client visits, if required.
  • Organized and flexible to meet business needs.
