About the Role

We are seeking a skilled Community Psychiatric Nurse to join our team at NHS Professionals Limited. As a Community Psychiatric Nurse, you will play a vital role in providing high-quality care to patients in the community, working closely with multidisciplinary teams to ensure the best possible outcomes.

Key Responsibilities
  • Conduct robust assessments of patients with mental health difficulties, identifying appropriate care pathways and ensuring the least restrictive option to meet their needs.
  • Signpost patients to appropriate care providers, including primary and tertiary care, and attend daily Red Board meetings to discuss patient care.
  • Complete documentation and planning care for patients, presenting cases to Multi-Disciplinary Teams for discussion around care pathways.
  • Liaise with colleagues from nursing, occupational therapy, medicine, psychology, and administration to ensure comprehensive care.

Requirements
