Job Overview

West London NHS Trust is seeking a highly skilled Community Psychiatric Nurse to join our team in Ealing Cognitive Impairment and Dementia Services. Main Responsibilities

  • Provide specialist assessments, reviews, and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS).
  • Ensure the successful implementation and roll-out of care for dementia patients.
  • Work as a core member of a multi-disciplinary team, collaborating with professionals to deliver high-quality care.
  • Act within the NMC Code of Conduct at all times.
  • Deliver care under the Care Programme Approach, working in partnership with individual service users and their families and carers to ensure the delivery of the care plan.

Requirements

  • Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
  • Evidence of continuous professional development.
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff.
  • Experience of managing clinical caseload, including carrying out initial assessments, treatment efficacy reviews, and risk mitigation assessments leading to crisis prevention.
