Job Title: Community Psychiatric Nurse

About the Role:

We are seeking a skilled Community Psychiatric Nurse to join our team on a flexible shift basis. As a Community Psychiatric Nurse, you will be responsible for providing high-quality care to patients in their homes or community settings.

Key Responsibilities:

  • Conduct robust assessments of patients with mental health difficulties
  • Identify appropriate care pathways and ensure the least restrictive option for patients
  • Signpost patients to appropriate care providers, including primary and tertiary care
  • Attend daily Red Board meetings and raise concerns to senior staff as needed
  • Complete documentation and planning care for patients
  • Liaise with colleagues from nursing, occupational therapy, medicine, psychology, and administration

Requirements:

  • A minimum of 6 months paid experience as a Community Psychiatric Nurse in an UK hospital environment within the last 3 years
  • Comply with NMC Professional Registration and keep up to date with current nursing practices
  • A full UK driving license and own vehicle (may be required depending on the Trust)
