Job Overview

West London NHS Trust is seeking a highly skilled and dedicated Community Psychiatric Nurse to join our team in Ealing Cognitive Impairment and Dementia Services. Main Responsibilities

  • Provide specialist assessments, reviews, and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS).
  • Ensure the successful implementation and roll-out of care for dementia patients.
  • Work as a core member of a Multi-Disciplinary Team (MDT) to deliver care under the Care Programme Approach.
  • Administer and monitor medication, including depots, prescribed to individual service users.
  • Broaden research and development skills through participation in local audit and research projects.

Requirements

  • Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
  • Evidence of continuous professional development.
  • Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
  • Experience of supervising staff and managing clinical caseload.
  • Ability to work within a Multi-Disciplinary Community Team and have an excellent understanding of assessment and management of risk.
