About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at the NHS, working in secure environments. As a Senior Practitioner, you will play a key role in delivering high-quality mental health services to individuals in custody.

Key Responsibilities
  • Support new prisoners during their first 14 days in custody, ensuring they access relevant healthcare services and manage their general health and wellbeing.
  • Conduct comprehensive assessments and develop individualized care plans that address the health and social care needs of patients.
  • Cover some work for the mental health in-reach team, including care planning, caseload reviews, and administering medication.
  • Collect and synthesize clinical history to ensure safety, continuity of care, and minimal re-assessments.
  • Ensure ongoing care for patients, with clear pathway management in place, and share information with relevant staff as needed.
Requirements
  • Experience working in mental health and learning disabilities, with a strong understanding of the Care Programme Approach.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multi-disciplinary team.
  • Knowledge of the Mental Health Act 1983 and experience working in secure locations.
  • Ability to develop initial mental health crisis plans and lead on identifying patients with changing needs or risks.
  • Collaborative working skills, with the ability to integrate with other specialist services and ensure continuity of care.
