Position Overview

Are you a dedicated professional seeking to enhance your expertise in supporting children and adolescents within the community?

We are in search of enthusiastic individuals who possess substantial knowledge and experience in engaging with children, young individuals, and their families.

The ideal candidate will demonstrate the capability to collaborate within a diverse team to implement specific elements of the Healthy Child Programme for those with Special Educational Needs and Disabilities (SEND). Your role will involve fostering positive connections with children, young people, and families, empowering them to engage, participate, take ownership, and make informed decisions.

As a collaborative team member under the guidance of a School Nurse, you will work alongside educational institutions to promote and enhance the health and well-being of children and families. This includes delivering specialized interventions and programs tailored to meet the needs of children and families identified as at risk of adverse outcomes or facing complex challenges.

HCRG is proud to partner with Barnardos as part of The Essex Child and Family Wellbeing Service (ECFWS), providing innovative and impactful health and social care services from pre-birth to 19 years for families in Essex.

Key Responsibilities

The following list outlines the primary responsibilities of this role, though it is not exhaustive:

  • Implement the Healthy Child Programme 0-19 pathway at various levels as directed by the Specialist Community Public Health School Nurse.
  • Design and facilitate engaging personal and social development sessions for youth aged 11-19, covering a variety of relevant topics.
  • Provide brief interventions and solution-focused support addressing issues such as emotional well-being, exploitation, sexual health, relationships, substance misuse, risk-taking behaviors, and anti-social activities.
  • Establish and maintain effective working relationships with young people and families at risk of social and educational exclusion, ensuring their personal and social education needs are recognized and addressed.
  • Collaborate within a multi-disciplinary framework with key stakeholders, including statutory and voluntary organizations, to identify and respond to the needs of young people and families.
  • Execute targeted health promotion initiatives through various channels, including social media and personal, social, health, and economic education (PSHE).
  • Engage with parents, guardians, and families to ensure they are actively involved in the support process and understand how to access assistance for themselves and their children.
  • Contribute to achieving key performance indicators (KPIs) to ensure compliance with the commissioned service specifications and provide written guidance as necessary.

Ideal Candidate Profile

The successful candidate will possess:

  • A degree or equivalent experience in Public Health or Public Health Nursing.
  • The ability to identify, assess, and manage risks while developing interventions to enhance health and well-being.
  • A strong understanding of the educational, employment, and social care needs of children and young people.
  • Excellent communication skills, capable of conveying complex issues in clear language to various audiences.
  • Experience managing a caseload of young individuals from vulnerable backgrounds, with the ability to maintain professional boundaries.
  • The ability to cultivate and sustain effective working relationships with partner agencies, colleagues, and service users.
  • A commitment to promoting and safeguarding the welfare of children and young people.
  • Familiarity with relevant legislation and guidelines, including the Children’s Act and Healthy Child Programme.

Proficiency in IT systems and a working knowledge of Microsoft Office applications (Outlook, Word, Excel) is essential.

A valid UK driving license and access to a vehicle for work purposes are required.
