Position Overview

Are you dedicated to making a significant impact in the lives of children and young individuals?

We invite you to contribute to the development of a pioneering keyworking support initiative for children and young people (CYP) in North East London.

We are seeking a Band 7 Lead Support Practitioner to enhance the new keyworking Service for children and young people (CYP) aged 0-25 years, who experience learning disabilities and/or autism. This service is designed to assist those with the most intricate needs, particularly those at risk of hospitalization or currently receiving inpatient care.

Applicants should possess direct experience in supporting families with children or adults facing complex challenges, demonstrating the ability to guide families and young individuals through healthcare systems and pathways. A strong capacity to empower parents, caregivers, and individuals while adopting a collaborative approach is essential.

We expect candidates to exhibit personal authority and confidence in advocating for the needs of our children and families, actively challenging the system to enhance outcomes for our service users. You will be responsible for implementing actions and support packages agreed upon by Education, Health, and Care Teams to prevent hospitalization and to create support frameworks that ensure safe and stable community-based care.

Key Responsibilities

  • Provide expert guidance and support, along with supervision and training to the team, ensuring the effective execution of the NHS 10-Year Plan on Keyworking.
  • Assist the Service Manager with assigned managerial responsibilities, including performance reviews and one-on-one meetings.
  • Deliver comprehensive clinical and/or professional supervision and leadership to the team.
  • Collaborate with the Service Manager to develop outcome-focused services.
  • Oversee a team of case managers responsible for the care and support coordination of children and young people on the Dynamic Support Registers.
  • Support and inform the Care (Education) and Treatment Review (C(E)TR) process for children and young people within the caseload.
  • Maintain a thorough understanding of relevant legislation, national standards, guidance, research, and departmental policies and procedures.
  • Keep accurate and up-to-date records of cases using management information systems.
  • Assist in executing specific development tasks as agreed with the Service Manager.
  • Prepare documentation for formal supervision under the guidance of the Service Manager and keep them informed of any potential challenges.
  • Chair and participate in case reviews and other meetings as required.
  • Collaborate and work jointly with colleagues and staff from other organizations, as appropriate, and represent service users in discussions with other agencies.

About NELFT

Joining NELFT NELFT places significant emphasis on ensuring that new employees are warmly welcomed and thoroughly inducted into the Trust. All new hires will commence their roles on the first Monday of each month and will undergo a comprehensive induction lasting up to two weeks, which includes mandatory training, systems training, and the provision of necessary equipment. During this process, new employees will have the opportunity to meet the executive team, senior managers, and participate in various drop-in sessions focused on engagement, health, wellbeing, and key processes. The induction will take place at our head office.

Probationary Period This position will be subject to a probationary period. Internal candidates are exempt from this requirement unless they are currently undergoing a probationary period or are bank staff.

High Cost Area Supplement - London This position also includes a High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum).

Qualifications

Essential

  • Degree-level education or equivalent experience.
  • Registered Nurse (RMN Mental Health, LD, RGN) or AHP or Dip SW, CSS or CQSW, or a CCETSW validated equivalent from another country.

Experience

Essential

  • Extensive experience working directly with children, young people, and their families facing complex, acute mental health conditions and challenging behaviors, including those who may be autistic and/or have learning disabilities.
  • Experience in managing staff to deliver services, including managerial and clinical supervision, performance reviews, recruitment, and compliance with development standards.

Knowledge

Essential

  • Comprehensive knowledge of the Health & Social Care legislative framework, priorities, policies, and guidance, including the Care Act 2014, the Mental Health Act, Mental Capacity Act, and NHS Long Term Plan.

Desirable

  • Strong working knowledge of the Care Programme Approach, Dynamic Support Registers, and Care and Treatment Review processes.

Skills

Essential

  • Ability to conduct thorough assessments of needs and formulate appropriate plans, including discharge planning and crisis management, as well as undertaking complex risk assessments when necessary.

Desirable

  • Well-developed interpersonal skills to build effective working relationships, resolve challenges, and support staff, service users, and caregivers on an ongoing basis.
