We are seeking a highly skilled and experienced individual to join our Hospital Discharge Team as a Coordinator. The successful candidate will play a crucial role in ensuring the smooth discharge of patients from our hospital, working closely with the multidisciplinary team to optimize patient flow and provide exceptional customer service.

About the Role

The Coordinator will be responsible for:

  • Coordinating the discharge process for patients, ensuring timely and efficient transfer to community services
  • Working closely with the multidisciplinary team to identify and address any barriers to discharge
  • Providing excellent customer service to patients, families, and healthcare professionals
  • Supporting the development and implementation of discharge pathways and protocols
  • Collaborating with community services to ensure seamless transfer of care
