Clinical Support Worker

We are seeking a motivated and dynamic Clinical Support Worker to join our friendly and supportive team at the Royal Devon and Exeter NHS Foundation Trust.
The successful candidate will work closely with our community colleagues to provide seamless transition of care between hospital and home or other receiving destinations. This will involve assessing and procuring onward care, giving advice, issuing equipment, and supporting with therapy needs for specific patient groups.
The role can be challenging and time-sensitive, but it is also rewarding and varied. You will have the opportunity to work in many different departments and wards throughout the acute site, liaising with various teams and specialists to facilitate appropriate discharge requirements.
As a Clinical Support Worker, you will also follow patients up in the community, providing further assessments and supporting them to remain safely at home. Basic administration tasks, such as answering telephone and email enquiries, collating information, and inputting referral information on our electronic health system, will also be required.
We are looking for a forward-thinking, dynamic, and motivated individual who can work as part of a team and under pressure. Experience from acute and community backgrounds is preferred, but not essential. Training and support will be provided to the successful applicant, including a robust induction period tailored to their needs.
