About the Role

We are seeking a motivated and dynamic Clinical Support Worker to join our friendly and supportive team at the Royal Devon and Exeter NHS Foundation Trust. As a Clinical Support Worker, you will play a vital role in supporting patients and their families, working closely with healthcare professionals to ensure seamless transitions of care between hospital and home.

Key Responsibilities
  • Assist with patient assessments and develop care plans to support patients in achieving their goals
  • Work collaboratively with multidisciplinary teams to ensure effective communication and coordination of care
  • Provide emotional support and guidance to patients and their families, promoting independence and self-management
  • Contribute to the development and implementation of care pathways and protocols to improve patient outcomes
  • Participate in quality improvement initiatives to enhance patient care and experience
Requirements

To be successful in this role, you will need:

  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong problem-solving and critical thinking skills
  • Knowledge of patient care principles and practices
  • Ability to work collaboratively as part of a team
