We are seeking a highly skilled and experienced Discharge Coordinator to join our team at the Royal Devon and Exeter NHS Foundation Trust. About the Role

The successful candidate will be responsible for coordinating the discharge process, working closely with healthcare professionals, patients, and their families to ensure a seamless transition. You will be required to:

  • Assess patient needs and develop individualized discharge plans
  • Coordinate with healthcare teams to ensure timely discharges
  • Communicate effectively with patients, families, and healthcare professionals
  • Identify and address any barriers to discharge
  • Monitor patient progress and adjust discharge plans as needed
Requirements

To be successful in this role, you will need:

  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
  • Ability to work effectively in a fast-paced environment

  • Previous experience in a healthcare setting, preferably in a ward environment
