**Pay Rate: £14.17 per hour PAYE
**37 Monday - Friday, normal working hours**
**This opening assignment is for 3 months**
**City: SOUTH SHIELDS**
**Office based, with the Integrated Hospital Discharge Team, and will cover Monday to Friday, however there may be times when shift work is required, including weekend and Bank Holidays**
**Enhanced DBS disclosure required**
The role of Hospital Discharge Coordinator is to support operational social care teams with ensuring people return home from hospital in a safe and timely manner, through successful partnership working with Health and Social Care. The role will take a proactive coordination approach covering all discharge pathways. There is a requirement of this role to carry out duties in the most flexible, effective, and efficient manner taking a home first approach
