Hospital Discharge Team Coordinator

We are seeking a highly skilled and experienced individual to join our Hospital Discharge Team as a Coordinator. The successful candidate will be responsible for coordinating the discharge process for patients, ensuring a smooth transition from hospital to community care.
About the RoleThe Coordinator will work closely with the multidisciplinary team, including healthcare professionals, social workers, and other stakeholders to ensure that patients receive the necessary care and support to facilitate a timely and safe discharge.
The ideal candidate will have excellent communication and organizational skills, with the ability to work effectively in a fast-paced environment. They will also have a strong understanding of the healthcare system and the ability to navigate complex care pathways.
Key Responsibilities- Coordinate the discharge process for patients, ensuring a smooth transition from hospital to community care.
- Work closely with the multidisciplinary team to ensure that patients receive the necessary care and support to facilitate a timely and safe discharge.
- Develop and maintain relationships with healthcare professionals, social workers, and other stakeholders to ensure effective communication and collaboration.
- Manage and prioritize tasks to ensure that discharge plans are implemented efficiently and effectively.
- Provide administrative support to the team, including data entry, record-keeping, and reporting.
The successful candidate will have:
