The service focuses on the assessment and management of individuals experiencing persistent symptoms following COVID-19 infection or suspected exposure. Our team provides a comprehensive array of interventions aimed at alleviating the effects of long COVID and empowering patients in managing their health effectively. These interventions are delivered through individual consultations, group sessions, and occasional home visits.
We welcome applications from candidates who possess strong communication abilities, experience in independent practice, and a genuine commitment to enhancing the quality of life for individuals living with long COVID as a chronic condition.
Work Schedule
The position entails a workweek of hours from Monday to Friday, with options for part-time and flexible arrangements available.
A valid means of transportation and the capability to travel within the designated service area are essential.
Key Responsibilities
- Actively participate as a member of the multidisciplinary team, contributing to exceptional patient-centered care.
- Serve as a positive representative of the service.
- Engage with patients in the designated service area.
- Develop a caseload of patients following initial evaluations conducted by qualified professionals.
- Conduct timely reviews and provide treatments for patients.
- Implement treatment and rehabilitation plans primarily in clinical settings, with some sessions conducted in patients' homes.
- Educate patients on self-management strategies for their long-term health needs.
- Adhere to care plans as outlined by the relevant healthcare professionals.
- Document outcomes and evaluations for review by qualified staff.
- Enhance clinical skills and competencies through ongoing training and supervision.
- Collaborate with colleagues to support continuous service improvement.

Required Experience
