### Role Overview

We are seeking a compassionate and caring Relief Support Worker to join our team. As a Relief Support Worker, you will provide encouragement, guidance, and support to individuals in their own homes, enabling them to live as independently as possible and make their own choices.

### Key Responsibilities

* Provide one-to-one support to individuals, building strong relationships and promoting independence
* Assist with personal care tasks, while prioritizing the individual's dignity and respect
* Encourage and support individuals to participate in their community, promoting social inclusion
* Work collaboratively with other professionals to ensure seamless support
