Physiotherapist - Community Rehabilitation Specialist

An exciting opportunity has arisen for a skilled Physiotherapist to join our Community Therapy Service as a Physiotherapist, working across community delivering rehabilitation in patients' own homes following discharge from an acute bed, or supporting Long-term condition management.
This is a maternity cover for 12 months, providing a unique opportunity to develop your skills and expertise in a supportive and enthusiastic team environment.
About the RoleAs a Physiotherapist, you will be responsible for assessing, intervening, and discharging a caseload of patients, working closely with the wider Multi-Disciplinary Team (MDT) and Integrated Therapy (INT) teams in community.
You will have the opportunity to develop an interest in frailty and long-term conditions, working with patients with a large variety of conditions and disabilities in their own homes and local community.
Main Duties- Provision of specialized assessment and treatment of patients referred to the service.
- Management of a caseload of patients with complex conditions, including long-term condition management and acute hospital discharges.
- Specialist assessment and treatment, including the prescription of essential equipment as part of the therapeutic intervention.
- Link with Primary Care Networks (PCN) and MDT teams, attending meetings as part of neighborhood team working.

Person Specification- Postgraduate therapy experience, including community and rehabilitation.
- Profession-specific Diploma/degree in Occupational Therapy or Physiotherapy.
- Health Professions Council Registration.
- Evidence of Continuing Professional Development (CPD) maintained in a portfolio, including attendance at recent postgraduate courses relevant to the clinical fields in rotations.
- Membership of the Chartered Society of Physiotherapy/Royal College of Occupational Therapy.
- Involvement in Special Clinical Interest Groups.
