About the Role

We are seeking an experienced Physiotherapist to join our Community Therapy Service as a Physiotherapist, working across community delivering rehabilitation in patients' own homes following discharge from an acute bed, or supporting Long term condition management.

Key Responsibilities
  • Provide specialised assessment and treatment of patients referred to the service.
  • Manage a caseload of patients with a variety of complex conditions, including long term condition management and acute hospital discharges.
  • See patients in their own homes, working with an MDT in other organisations.
  • Carry out specialist assessment and treatment, including the prescription of essential equipment as part of the therapeutic intervention.
  • Play an important link with PCN and MDT teams in attending meetings as part of neighbourhood team working.

Person Specification
  • Post graduate therapy experience to include community and rehab.
  • Profession specific Diploma/degree in Occupational Therapy or Physiotherapy.
  • Health Professions Council Registration.
  • Evidence of CPD maintained in a portfolio, including attendance at recent postgraduate courses relevant to the clinical fields in rotations.
  • Membership of the Chartered Society of Physiotherapy/Royal College of Occupational Therapy.
  • Involvement in Special Clinical Interest Group.
