Community Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our community rehabilitation team at East Lancashire Hospitals Trust. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality rehabilitation services to patients in a community hospital setting.
About the Role- Deliver specialist assessment, treatment, and intervention to patients with a wide range of conditions and disabilities.
- Manage a caseload of patients on a community rehab ward and coordinate onward referrals and discharges.
- Work collaboratively with other healthcare professionals to provide comprehensive care.
- Develop and maintain a portfolio of continuing professional development, including attendance at postgraduate courses and conferences.
- Postgraduate therapy experience, including community and rehabilitation settings.
- Profession-specific diploma or degree in Occupational Therapy or Physiotherapy.
- Health Professions Council registration.
- Membership of the Chartered Society of Physiotherapy or Royal College of Occupational Therapy.
